Hours to be agreed with post holder set against the backdrop of the needs of the service and in consultation with the Service Manager as to the most effective operating hours
Contract: 12-month fixed term contract
We are looking for a number of SMI Practitioners & one Lead SMI Practitioner. And Bank Peer Support Workers
There are x5 SMI Practitioner posts, 1 per borough – Wandsworth, Croydon. Kingston Richmond and Merton. Lead SMI Practitioner x1 in Wandsworth, and Bank Peer Support Workers –sessional / on an ad hoc basis.
You work as a team across five South West London boroughs, along with a bank of Peer Support Workers, each with a specific borough focus to work with existing provision to reach a target of 5% additional people on SMI register accessing their annual health checks.
The roles will be community based and outreach focused, requiring close working with Primary Care Networks, working flexibly, support patients to remove barriers in accessing their annual health checks, working around existing provision and responding to the needs of individuals. The work may involve a one-off intervention, such as a telephone call to prompt and support someone to access their check, to several contacts, including supporting them to the appointment in person, sending messages introducing the service, arranging a Peer Support involvement to enhance engagement in achieving a successful annual health check, and liaison with practices and clinics.
A personalised, proactive and flexible way of working is required for these Posts.
Joined up approach – building relationships with existing partners and targeting the 5% of hardest to reach individuals.
Postholders will also signpost to other services when needed.
The Lead Practitioner Postholder will line manage the Practitioners and oversee day to day responsibility of the service, ensuring adherence to policies and procedures, supporting the team of Practitioners in meeting service requirements, activity and targets.
They will work within the Southwest London Local Mind Partnership (Mind in Brent, Wandsworth and Westminster, Mind in Croydon, Mind in Kingston and Mind in Richmond). Working across the boroughs of Wandsworth, Croydon, Kingston, Richmond and Merton, working with Primary Care Networks, GP practices and Health clinics and existing SMI providers across these localities.
In partnership with local Minds, we are seeking to fill several Sessional Peer Support Worker roles. The SMI Checks project is for one year with the potential to extend beyond this initial period. Please note they are based at and in different Boroughs.
